Introduction
I grew up in a small farming community in southern Illinois, the youngest of three children. My two older sisters are in the arts; one is a dancer and the other a writer. Naturally, with these influences, I was actively involved in theatre and visual arts throughout school. I went to college thinking of becoming an engineer like my father. I enjoy mathematics and science, especially the idea of puzzles and putting things together.
However, while on a medical mission trip to see the impact medicine can have on people, I decided immediately I wanted to be a physician. I entered medical school wanting to be a family physician; that is the role I admired while growing up in a small town. I liked the thought of being involved in patients’ lives for their longitudinal needs. Also, I appreciated the family doctor’s active involvement in so many aspects of the community and its needs.
I was sitting in the anatomy lab during the first year of medical school when I fell in love with the complexity of human anatomy and human form. I knew at that moment I must be a surgeon. During a plastic surgery rotation, I had the opportunity to work with a surgical master. While watching the precision and the way he cared for his breast reconstruction patients, I knew I wanted to be a plastic surgeon. I saw in the way he cared for these women, that as a plastic surgeon I could be involved in my patients’ longitudinal care much as I envisioned as a family physician.

A breast lift and reduction are similar procedures and often confused as the scar patterns can be identical. If you went the strict definition in plastic surgery textbooks a procedure becomes a reduction when you take more breast tissue (in the book 500g). Also a reduction is typically performed for functional reasons (pain, grooving, rashes) and is covered by insurance whereas a lift is for cosmetic purposes and is paid out of pocket.
IN the real world; a lift is a cosmetic procedure that is not performed for functional reasons (to eliminate pain etc). Though one could argue that a breast reduction ALSO has a cosmetic component to it as well.
The real question you may be asking is; will my procedure be covered by insurance still. To be covered by insurance; your surgeon will need to document you are having symptoms (neck pain, back pain, shoulder grooving), that you have tried conservative measures to eliminate symptoms (weight loss, NSAIDs, PT, chiropractics etc). Then we submit clinical photographs and document the expected amount of tissue in weight to be removed. The last part may pertain to your question as insurance providers use a system called the Schnurr scale in which for a given body surface area, your surgeon needs to remove a certain amount of tissue.
In order to optimize your long term results I would get to your goal weight before undergoing any procedure. After you reach your goal weight, meeting with a board certified plastic surgeon should be able to answer your questions.

First of all, I am sorry to hear that this is occurring. Skin flap necrosis following a facelift is a known complication and happens a certain percentage of the time despite your lack of risk factors and your surgeon's experience. The treatment for this complication is to leave the area alone and apply antibiotic ointment (neosporin). It should heal in on its own and often will leave a satisfactory appearance. Debridement would be overly aggressive for this issue.

It is hard to determine what is occurring with the sound you are hearing. It may be a temporary issue or could be due to capsular contracture. The first thing I would have my patient is to have a mammogram/ultrasound to determine if the implant is intact without any signs of rupture or capsular contracture. I would also encourage you to schedule a consult with your board certified plastic surgeon to evaluate for any clinical signs of capsular contracture.

Should I now have eye corners stitches after 2 failed eye lift and revision? 1st eye lift Aug 2022. Second was Oct 22 due to eye medical issues. Now the ectropion eyes are not improved and he now wants to stitch the corners. Eyes never healed. Still having tears, dry eyes and light sensitivity…

Ectropion is a known complication after lower eyelid surgery; particularly in revision surgery and patients with poor lid support (negative vector, older patient, etc). This complication can lead to chronic dry eyes and photophobia. To combat this, massage can be attempted but is not always successful. If diagnosed preoperatively measure can be take to improve lower lid support (corner stitches also known as cantopexy or even suturing eyes closed with Frost sutures).
That being said, ectropion and dry eyes can still occur. Proper treatment for this can include canthoplasty, canthopexy, or a tarsal strip; all of which can loosely by described as 'corner sutures'.
I would be sure to seek consult with a board certified plastic surgeon who specializes in aesthetic surgery; a member of The Aesthetic Society demonstrates such a surgeon.
