Scarring is a process of skin healing. It is always permanent and visible on the face. I can only change some appearance of the scar doing that by excision. The new scar will be the next step. Nowadays I'd rather inject fat giving some support to the skin and I also inject intradermally the fat to rebuilt the scar. The majority of patients have improvement and higher satisfaction with the fat transfer than excision.

Nowadays I give 2 choices to my patients. There are 2 different techniques. I can place implants to make look the breast fuller but I can also use a technique of fat transfer to the breast. In my 6 years experience, I know that fat transfer gives less long term complications. The rate of satisfaction after this procedure is very high.The recovery is easier than an implant placement. We can adjust breast shape 1-2 times with fat transfer and repeat that procedure with the next 5 or 8 years to improve the shape of the breast when they start to hang again with aging.

In most of cases of abdominoplasty, I repair muscles in the wall of the abdomen. The result of such a suture gives a better cosmetic result and a flat tummy. In young patients who want to have more children or are not sure to go for another pregnancy, I don't place sutures on muscles. In the future when they are done with the "family plan" I proceed with muscle repair.

Every surgeon needs to talk to their patient about limits in our life. We are doctors and our Code of Ethics has to be shown in front of the patient for his/her own benefit. We give advice about techniques and limits in plastic surgery procedures. We want our patients to feel good in their skin and we don't want to harm them. As a surgeon with 25 years of experience I feel the need to educate my patients and give them my support to help with difficult decisions.
