No plastic surgeon should promise nor predict cup size. There are no standard sizes, you try on three different brands that fit, they’ll be different! Your insurance company sets the criteria for the amount to be removed for coverage. What you list as the Schnur scale is about right. I am not sure where your surgeon’s estimate comes from but probably close. The best way to estimate how you may look after a reduction is to have the surgeon “ pinch” your tissue to simulate the reduction and see if you like the size. Don’t worry about the weight as long as it covers the insurance minimum. You being petite and a DDD, 400 Ms should still leave you a nice size. Discuss this in front of a mirror with your surgeon.

Radiation to the breasts limits what should be done to them surgically as the tissues are permanently damaged. Whatever technique your surgeon chose, should obtain an improvement for you. It will take at least 6 months to see the result shape up. After that nothing should be done for at least a year. Again, radiation is the game changer here limiting what could/should be done.

Staging these procedures is safest, with lower complications and a better chance of choosing the appropriate implant for you! Your current size makes me question if you need an implant at all. A lift should create a nice change for you. If, afterwards, you desire a larger size or more upper volume, a small implant can be chosen. Waiting will allow your surgeon to find the right size.

It depends on your breast anatomy! Implants must be centered beneath the nipples ( if they are not sagging). If your breasts were facing " out" then you may not be able to get the cleavage you desire without getting too big! If the implants do migrate too far to the side, a revision can be done to move them more centrally, but again, they must be centered beneath the nipples.
