A LOT has changed since you were first faced with this acne scar concern. Acne scars can be "ice pick" or general depressed facial scars. They can be associated with hyper pigmentation or hypo pigmentation. The problem of your acne scars still remain now that you are 47 y/o vs. 25. However, you may also have "extra" skin that may make these scars appear worse. While a facelift should reduce the extra skin on your face and INITIALLY make your acne scars appear better due to the post operative swelling, the acne scars will remain. The face lift swelling will subside and the acne scars will be evident. I think most board certified plastic surgeons will feel the same way. Mini-facelift, thread lift, deep plane facelift, it does not matter. The problem of acne scarring is within the skin that remains. There may be an improvement in skin contour as there is less extra skin but I would not recommend this to my SF Bay Area patients. There are good options to treat acne scars. For my patients in San Francisco, I initially suggest "skin fillers" which can provide a temporary relief for contour issues. Restylane, Juvaderm are good example of such fillers. Fat injections are an another option but require the additional time and cost of harvesting the fat. Laser skin resurfacing, chemical peels alone or combined with skin fillers is a great option for depressed scars and may also deal with any unwanted dark pigment on your skin. Sometimes a scar revision may need to be performed prior to laser skin resurfacing. While you have waited to deal with you acne scars, there may be better options now!

Your question is more related to an entire facial rejuvenation vs. just an eyelid procedure. So to break down your question - The extra skin on your eyelids can be removed safely. It also often involves removing or repositioning "extra" fat around your eyelids. Sutures are usually removed in one week. The swelling subsides rapidly initially and the final result may take 6-12 months. In terms of your other concerns, this would require a "face lift" depending on your expectations. There are some non-invasive methods of neck skin tightening but they do not compare consistently with the results of a surgical procedure. As a board certified plastic surgeon in San Francisco, I have been very interested in reducing the recovery time of a face lift procedure. The recovery process, which incorporates the reduction in bruising and swelling after surgery has been reduced in my patients by using a laser to dissect the tissue at the time of the face lift procedure vs. scissors or knife. The laser seals blood vessels as they are divided, which results in less initial post operative bruising. The less bruising initially, the shorter the total recovery period. Other important items factor into one's recovery period - the patient's age, health, number of procedures performed during the surgery and the post operative protocol. Keeping one's head elevated above their heart, initial cooling, resting vs. trying to return to normal activities immediately can factor into the length of the recovery period.

When performing rhinoplasty surgery in San Francisco, often know as a "nose job" the best advice I provide my patients is to communicate with me BEFORE their procedure so that we are on the same page in terms of expectation. During the pre-operative consultation, all of the potential side effects should be discussed with you. Your pre-operative health is important and make sure you let your ASAPS doctor know of all medications you are taking, even over the counter (OTC) items. This information will be helpful as part of your pre-operative meeting. In terms of the healing process, a great deal depends on how much "work" / changes will be afforded you. The more surgical work, the longer the recovery time is usually. As a cosmetic surgeon in San Francisco, it is hard for me to advise you regarding your specific care after your rhinoplasty. In general, the nasal splint is removed in 7 days, and then additional tape may or may not be applied to your nose. I recommend nasal exercises for my patients to insure they maintain a straight nose. The visual improvement in your nasal shape will occur rapidly initially but the final outcome may take 12- 16 months to be evident.
