Your description seems to be one of normal skin laxity that everyone has when they bend at the waist. True, your skin may not have the elasticity it had prior to you pregnancies, but if you don't have excess skin, no procedure could be recommended. Stretch marks are, unfortunately, permanent in spite of claims made that various external treatments can eliminate them. You best next step would be to contact a board certified plastic surgeon in your area and have an exam so the best option could be suggested to you.

Your description sounds like you have enlarged areolas A breast augmentation would not help this, as the only thing a breast augmentation does is to give you a larger version of the breasts you already have. To decrease the size of the areolas there is a procedure called a Donut mastopexy which may work well for you. You should see a board certified plastic surgeon in your area as your first step to correcting your concern.

Your description sounds like too much fat was removed, especially directly under your chin and did not correct the loose muscle and skin which you had. The best correction to improve the contour of your neck is a neck lift/face lift where the remaining skin and fat of your neck are elevated, the bands of platysmal muscle of your neck is sutured together in the midline and by doing so this will help correct or possibly completely correct the "ditch" below your chin. The neck contour is further smoothed by tightening the platysmal muscle near your ear. The skin is then smoothed over this newly created contour and the excess removed. This should correct the bands and the excess skin. I recommend the suturing of the platysmal muscles to fill the defect below your chin because this tissue has an excellent blood supply and a very high chance of all the volume remaining while fat grafting is not predictable as to how much volume is retained, especially if the fat is placed into an area which has prior surgery. See a board certified plastic surgeon to discuss this and ask to see their before and after photos to see their results on patients whose necks are like yours.

The tissues which lie between your tummy muscles are stretched and this is what results in your abdomen buldging forward. These tissues don't have any elasticity and won't correct themselves. They need to be sutured together. The sutures should hold the tissues well and the results should be permanent. The muscle repair, the correction of the belly button and some tightening of the skin could be done by an incision low on your abdomen and should be very well hidden. At the same time it should be no problem to do a breast augmentation and a nipple reduction to complete your "Mommy Makeover".
