This male patient presented with nasal obstruction, aesthetic concerns, and left-sided deviation. Evaluation showed septal deviation in areas II and III, a left bony spur, and hypertrophy of the inferior and middle turbinates. He underwent a primary open septorhinoplasty with dorsal refinement, alar resection and SEG plication, plus a shield graft. Functional procedures included septoplasty, release of the bony spur, and luxofracture of the inferior and middle turbinates. The surgery was done under sedation, lasted 1 hour, and had no complications. Postoperatively, the patient reported improved breathing and was very satisfied with the more balanced, natural appearance of his nose.
